LIST FOR PENSION CASES
A.
FOR RETIREMENT CASES
[Form numbers as laid down in CES
(Pension)’s Rules, Swami’s
compilation]
1.
Form 3 showing details of Family.
2.
Form 5 Application for pension.
3.
Form 7 Form for assessing Pension/Family Pension and gratuity.
4.
Service Book duly completed as on date of submission.
5.
Two-specimen signature duly attested by a Gazetted Officer or Two
slips bearing the left thumb and finger impression duly attested by a
Gazetted Officer in case the pensioner is not literate enough to sign
his/her name.
6.
Two copies of passport size photograph with wife or husband
(either jointly or
Separately) duly attested by Head of the Office.
7.
Two slips showing the particulars of height and identification
marks duly attested by a Gazetted Officer.
8.
In case of revision in pay causing revision in Pension, Gratuity,
Commutation etc revised fixation of Pay along with calculation sheet of
pension etc. should be submitted along with payment sanction authority.
All relevant documents may be countersigned by the GMT/TDM as the case
may be in token of acceptance of the statement/ documents etc. unless
any subordinate officer is declared as “Head of the Office”.

B. FOR DEATH CASES
IN
CASE OF DEATH OF A GOVERNMENT SERVANT FOLLOWING SHOULD BE SUBMITTED TO
COMMUNICATION ACCOUNTS OFFICE THROUGH AO (TA) OF THE SSA CONCERNED. ALL
DOCUMENTS ARE TO BE COUNTERSIGNED BY HEAD OF THE OFFICE OR ANY OTHER
OFFICER SO ENTRUSTED BY HEAD OF THE OFFICE.
1.
Form 14 -- Application for grant of Family Pension.
2.
Form 12 – Application for grant of death gratuity.
3.
Form 18 – For assessing and authorizing payment of family
pension and death gratuity when a govt. servant dies while in service.
4.
Service Book indicating date of death.
5.
Two specimen signatures or left hand thumb and finger impression
of the claimant or guardian ( in case of minor) duly attested.
6.
Two copies of passport size photographs of the claimant or
guardian duly attested.
7.
Two copies of descriptive roll of the claimant or guardian duly
attested indicating height and personal marks.
8.
Postal
Address of the claimant.
